This will take approximately up to 5 minutes. Some of the following questions will ask for personal data (e.g. age, zip code, health data). Due to the nature of the questions raised, we would like to point out that all information provided by you will be processed in line with HIPAA laws and the EU General Data Protection Regulation (GDPR). Please note that you can terminate the questionnaire at any time without submitting any data. Upon successful completion of the following questionnaire, you will be asked to provide your contact information which will be forwarded to your clinical site and the staff will contact you with further information.
